Community Based Care/Personal Care Screenings
The division conducts screenings for residents that require public assistance in payment for Community Based Care. A family services specialist and public health nurse will meet with the individual to determine if they qualify for home based care through the Medicaid Waiver program. Applicant must also meet financial requirements by applying for Long Term Care Medicaid. Contact the Division of Aging and Adult Services at 703.746.5999 to request an assessment.
Assisted Living Facility Screenings
The division conducts screenings for residents that require public assistance to assist in payment for assisted living facilities.
