Long Term Care

Care Coordination is available to assess the needs of residents age 60 and over to connect them with appropriate services.

This service includes:

• In-home assessments
• Case management
• Coordination of services
• Information and referral and assistance

There is no charge for this service. It may take up to one month to arrange for an initial home visit.
