I'm concerned about CARBON MONOXIDE in my home.
For general information about carbon monoxide and ways to protect your family, call the T&ES Office of Environmental Quality at 703.746.4065. If you ever experience symptoms such as dizziness or shortness of breath which you believe could be caused by CO buildup in your home, do not hesitate to call 911.
How do I obtain a DEATH CERTIFICATE?
If the person died within 10 years, you may call the Health Department at 703.746.4991 to get information about applying for a death certificate. You may also visit the Health Department’s 4480 King Street location, 2nd Floor, and pick up a death certificate form. If it has been over 10 years, call the VDH Office of Vital Records in Richmond, Virginia at 804. 662.6200.
From Old Town Alexandria: Take King Street (Rte. 7) north past TC Williams High School. Cross Braddock Road, and pass Bradlee Shopping Center and the exit for I-395. The Health Department is located a ¼ mile on the left, 4480 King Street, on the corner of King Street and S. 28th Street. On-street parking is available.
From West End Alexandria: Take Duke Street East, turning left on Quaker Lane. Take Quaker Lane past Seminary Road and Braddock Road, turning left on King Street (Rte. 7). Pass Bradlee Shopping Center and the exit for I-395. The Health Department is located a ¼ mile on the left, 4480 King Street, on the corner of King Street and S. 28th Street. On-street parking is available.
From the Capital Beltway: Take the I-95 N exit (Shirley Highway). Get off on the King Street/Rte. 7 North exit. Get over into the left lane. The Health Department is located a ¼ mile on the left, 4480 King Street, on the corner of King Street and S. 28th Street. On-street parking is available.

Can I get FLU OR PNEUMONIA SHOTS at the Health Department?
Influenza vaccine is available in the AHD Immunization Clinic during the flu season. Flu shots are generally offered beginning in late October.
Pneumonia vaccine is available year-round. There is a charge for these vaccines unless you have Medicare Part B. For more information, please call 703.746.4996.

I will be traveling out of the country and need shots. Do you provide FOREIGN TRAVEL immunization?
The Alexandria Health Department no longer offers immunization for foreign travel. For information on sites in Alexandria where you can get travel immunizations, please call 703.746.4980.

My child needs a PHYSICAL for School/Sports/Work:
You must live in the City of Alexandria and be financially eligible for your child to receive a physical examination at the Alexandria Health Department. Physical examinations are provided for children from birth until 5 years of age, and up to 11 years of age for school entry. Please call 703.746.4940.
Children 12 to 19 years of age may receive health care through the Teen Wellness Center, 703.746.4776, located at 3330 King Street at TC Williams High School. Hours for the Teen Wellness Center are 10 a.m. to 5 p.m. Walk-in or call for an appointment.
I ate at a RESTAURANT and got sick.
If the restaurant is within City of Alexandria limits, please call the Division of Environmental Health at 703.746.4910. Please be able to provide the following: exact name and address of the establishment, time and date of visit, all items of food eaten and symptoms, if any.
I have a bad ROACH or RAT problem in my apartment.
The Health Department does not handle inspection of homes or apartments. Contact your property manager. You can also contact the City of Alexandria's Division of Fire & Code Administration at 703.746.4200.
My child needs SHOTS. When can I come in and what is the cost?
All required shots for children are free. However, if you have private medical insurance, there may be some children's immunizations which are not required for school admission that you must obtain from your private doctor. Children's shots are offered 3 times a week at the Health Department’s, Main Office, on the following days:
- Mondays: 1 p.m. to 4 p.m.
- Tuesdays: 1 p.m. to 4 p.m.
- Wednesdays: 8 a.m. to 11 a.m.
You do not need an appointment. Please bring your child's immunization record(s) with you and present them at the time of registration. This will help us determine what shots your child needs. If you do not live in the City of Alexandria, it may be more convenient to obtain shots in the county in which you reside. Please call 703.746.4980 for more information.
I need a copy of my child's SHOT/IMMUNIZATION RECORD.
If your child received shots at the Alexandria Heath Department, we can provide you a copy of your child's record (there is a fee for this), or we can fax it to your physician. Please call 703.746.4980 for more information.
Symptoms of tuberculosis (TB) may include a cough that lasts for more than 2-3 weeks; coughing up blood; night sweats; loss of appetite; loss of weight; and fatigue. Please call 703.746.4960 for more information or to speak to a nurse.

I need a TB TEST.
The TB Clinic provides evaluation, treatment and preventive treatment programs for tuberculosis (TB).
Walk-in TB risk assessment and TB testing FOR ADULTS ONLY (19 years of age and older; children must attend the Immunization Clinic) is offered at the on the following day:
- Tuesdays - 8 a.m. to 11 a.m.
